Dialectical Behavior Therapy (DBT), developed by Dr. Marsha Linehanis a recognized form of psychotherapy designed to support individuals struggling with borderline personality disorder (BPD). In my practice, I use DBT as part of the modern “third wave” of cognitive-behavioral therapies, co-developed by Linehan and Steven Hayes, to offer patients precise and empathetic support in achieving emotional harmony.
DBT is based on solid scientific evidence, initially confirmed in studies involving individuals with BPD and suicidal tendencies. Subsequent analyses have demonstrated its effectiveness in addressing a variety of challenges, such as depression in older adults, bulimia, binge eating disorder, suicidal tendencies among adolescents, and difficulties associated with antisocial personality disorder.
The American Psychological Association (APA) recommends DBT as the leading method for treating BPD, recognizing its ability to reduce hospitalizations, suicidal acts, and treatment dropouts.
This method harmoniously combines cognitive-behavioral therapy techniques—such as thought modification, emotion regulation, and exposure—with mindfulness, acceptance, and dialectical processes. As a result, it supports patients in developing the skills to cope with intense emotions and create a more balanced life.
The standard DBTmodel, whose effectiveness is best documented, includes four key components:
- Individual psychotherapy: weekly sessions during which current challenges are discussed, with a focus on priorities such as eliminating life-threatening behaviors (e.g., suicide attempts, self-harm), improving engagement in therapy, and enhancing quality of life.
- Skills training: individual or group sessions where patients learn practical strategies for managing emotions, interpersonal skills, mindfulness, and stress tolerance, supported by exercises and homework assignments.
- Phone coaching: the ability to contact a therapist during moments of crisis to apply newly learned skills before resorting to destructive behaviors, providing support at critical moments.
- Consultations for therapists: regular team supervision meetings that ensure therapy remains consistent with DBT principles, offering the highest standard of care and mutual support among specialists.
My approach, which integrates DBT with other methods such as Gestalt therapy and positive psychology, is always tailored to individual needs and subject to regular supervision. DBT is an invitation to conscious self-improvement, which paves the way to greater emotional stability and a fulfilling life.
